You can purchase the XTERN out-of-pocket from this site and have your insurance provide reimburse you. To find out you qualify with your insurance plan, please call your provider and ask the following 3 questions:

  1. Am I covered for code L-1951?
  2. If so, for how much?
  3. What form do I need to submit for reimbursement? I know I need to include a doctor's prescription and the superbill from the distributor I will purchase from, anything else I need to provide?
